Unit Re-accreditation Process

Application for Re-accreditation of an AGES Accredited Training Unit
This application form is intended to document your unit’s ongoing capability to train Advanced Gynaecological Endoscopic and/or Laparoscopic surgeons, aligned with the Australasian Gynaecological Endoscopy and Surgery (AGES) Advanced Training Program (AATP).

This form must be completed for the re-accreditation of your Training Unit by AGES, or if there are significant changes affecting the unit's ability to deliver the AGES Curriculum during the accredited period e.g. significant staff change.

Submission Timeline
Applications should be submitted 3 to 6 months prior to the expiration of your unit's current accreditation. Incomplete applications will be returned for additional information, which may delay the processing.

Review Process
Re-accreditation decisions are based on the information provided in this application. An interview may be scheduled to clarify the documentation. Educationalists familiar with the AATP will make an initial recommendation to the AGES Education Committee, which may result in one of the following outcomes:

1. The unit should be re-accredited for 5 years from the expiration date of the current accreditation.

2. The unit should be re-accredited for a shorter period (e.g., 6 months) pending submission of additional evidence to meet AGES Curriculum standards.

3. The unit should not be re-accredited.

The AGES Education Committee will review the recommendation and make a final decision, with recourse to the AGES Board if necessary. Applicants may appeal the decision by emailing the Chairperson of the AGES Education Committee at education@ages.com.au .

Cost of Application
A non-refundable Application Fee of $1,500 + GST is required, payable to the independent reviewer via AGES. To request an invoice, please email education@ages.com.au.

Required Documentation
Please submit the following documents with your application. Use the buttons below to access the forms:

- Completed Supervisors Agreement and Requirements for each supervisor (including the Director).
- Completed Supervisor’s Rubric for any new supervisors.
- Completed Clinical Timetable for each trainee.
