Terms & Conditions
All Trainees must sign and submit the AGES Trainee Terms & Conditions by emailing the completed form to the AGES Education Coordinator:

AGES Policies
Registration Form
You are required to complete and submit the below form once you have been appointed as an AGES Trainee.
This online form will be automatically submitted to the AGES Secretariat.
Trainee Assessment Record (TAR) Forms
You are required to complete and submit your Trainee Assessment Record (TAR) at the end of each Semester over the course of your training. This must be done using the specific link that will be sent to you to enable you to complete your TAR on REDCap each semester.
TAR Supervisor's Report Form 2 is to be completed by each of your Training Supervisors and will be automatically submitted to the AGES Education Coordinator.
The AGES Education Coordinator collates the information from the Supervisor's Reports into the Director's Summary Form 3, and forwards the link to the Director for sign off.
Assessment of Procedural Skills (APS) Forms
At the completion of your training, you are required to submit two Summative APS forms of which one must be a Laparoscopic Hysterectomy.
These online forms will be automatically submitted to the AGES Secretariat.
Other AATP Requirements
As per the above Trainee Guidelines, trainees are required to also complete additional requirements for their training other than the Training Assessments (TARs) and Procedural Assessments (APSs).
Please note – The Advanced Training Workshop held each year prior to commencement of the ASM, does not need to be submitted for approval as it is funded by AGES and is a pre-requisite of your training requirements.
Exemption & Extension Requests
Extended Leave of Absence
AGES fellows should consider (when making leave applications) that AGES is not an employing body and that whilst AGES offers places in the AATP training program, AGES is not able to guarantee any employing hospital or Training Unit will approve any leave request made.
